Parent Trainer/Case Manager - Behavioral Health

Job Description

 ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

1. Facilitates Parent Education/Support Groups covering topics such as early childhood brain development, infant/childhood ages and stages, parenting and nurturing techniques and skills, developmental delay and early intervention education, dealing with difficult behaviors and healthy nutrition.
2. Conducts developmental screenings and generates related reports. Utilize observations and screenings to develop education and intervention plans with families.
3. Coordinates referrals to support programs such as the Genesis, Public Health Nursing, Head Start, WIC, Behavioral Health Services and other services as needed.
4. Refers to Early Childhood Special Education Services for evaluations and services as needed.
5. Performs case management activities such as following up on attendance and compliance of teen parents with their prenatal and/or well child visits.
6. Conduct home visits and family sessions to enhance learning and growth.
7. Performs a variety of screening and assessment functions including, but not limited to: social history, personal and/or family resources and substance abuse
8. Provides instruction and/or referral to participants in obtaining and utilizing medical ,socio-economic, mental health and related resources
9. Collaborates with patient, family members and other involved parties to assess barriers to attainment of goals and/or resources
10. Utilizes and disseminates information regarding availability of alternative resources including, but not limited to: VA, BIA, I H S, state, county, Tribal. Social Security and private resources
11. Works closely and cooperatively with other agencies and organizations acting as point of contact for communication/coordination of care between providers
12. Receives referrals and initiates follow up contact to determine needs, and explore appropriate resources
13. Provides urgent and emergency intervention to triage situation identifying appropriate referral and/or resources
14. Participates in departmental orientation, on the job training and quality assurance programs/initiatives
15. Participates in a variety of department and hospital educational programs to maintain current skill and competency levels; identifies and discusses performance or training needs with Supervisor

Skills/Requirements

 JOB SUMMARY:
Parent trainer / Case manager will be responsible for recruiting, engaging and working with teen parents to meet the needs of their very young children. The case manager will teach, model and coach participants in parenting skills and family communication. Case manager will assess the child, parent and their families for additional needs and facilitate referrals to appropriate services.


MINIMUM QUALIFICATIONS:
1. Bachelor’s degree in Social Work or closely related field; equivalent combination of education and experience; Master’s degree preferred
2. Current State of Arizona certification as a Social Worker; CPR certificate within 30 days of hire
3. Must have at least 2 years previous case management experience; Native American population preferred
4. Training and experience in implementing best practices related to infant mental health, child development and parenting.
5. Current valid State of Arizona driver’s license; fingerprint clearance to work with children upon hire
